History lesson
While the concept of gender dysphoria is a modern term, the recognition of more than two genders is far from new. Many cultures throughout history have acknowledged and celebrated gender diversity, with some societies recognizing multiple genders beyond the binary of male and female.
In Indigenous cultures across North America, for example, the concept of Two-Spirit people has long been a part of their social and spiritual fabric. Two-Spirit individuals are recognized as embodying both masculine and feminine qualities, and they often hold important roles within their communities, such as healers, mediators, or spiritual leaders. The term “Two-Spirit” itself was coined in the 1990s to encompass these diverse gender identities within Indigenous cultures, although the concept has existed for centuries.
Similarly, in South Asia, hijras have been recognized for thousands of years as a distinct third gender. Hijras, who may be intersex, transgender, or eunuchs, have played significant roles in religious and cultural ceremonies, often seen as bringers of blessings or good fortune. Despite facing discrimination in many parts of society, hijras are an enduring part of South Asian culture, with historical roots that date back to ancient texts and traditions.
In Samoa, the fa’afafine are another example of a recognized third gender. Fa’afafine are individuals who are assigned male at birth but embody both masculine and feminine traits, often taking on roles traditionally associated with women in Samoan society. Fa’afafine are widely accepted and respected, illustrating how some cultures have a more fluid understanding of gender roles and identities.

Did you know?
Gender dysphoria can look different for each person. Some people might feel a constant discomfort with their assigned gender, while others experience it only in specific situations, like when using a public restroom or seeing their reflection. This condition also affects people across the gender spectrum, including those who are transgender, non-binary, or gender non-conforming.
Recognizing that gender dysphoria varies widely is key to providing the right support. There’s no one-size-fits-all approach, and understanding these differences can help ensure that each person gets the care that’s right for them.
